Cecil County 4-H Forms

Here's where you will find all of the up-to-date forms to accompany entries into the spring and summer events such as Eastern Shore Spring Show and the Breeder's Fair, as well as the upcoming Cecil County Fair and Maryland State Fair.

New 4-H member enrollment forms are to be completed by the enrolling member and then signed by a parent/guardian and the club leader, before they are sent to the Extension Office. The Donor Form is used for donations to the annual Benefit Auction as well as for all donations made to the 4-H Youth Development Program throughout the year.
